From experience, quite a lot of Americans use the drive-thru and then park in the McDonalds lot to eat the food. When I lived there it didn't seem abnormal. Now it does!
Didn't go to a McDs drive-through in America, but my experience of In-N-Out Burgers in LA was that there is no eat-in space (or very minimal), so you either ate in the car park or the outdoor benches or drive home.
